If you do not have it you may download it for FREE at Adobe.com Form Description Instructions Firearms Licensing Application Application for FID and LTC new and renewal Print out and complete this form and bring it to Firearms Licensing Officer at APD on the first and third Tuesdays of the month between the hours of 4pm and 7pm. (Photos are NOT needed) Housewatch Request form for 14 day housewatch. Print and complete this form and drop it off at the station before you go away. Housewatches are only good for 14 days unless renewed prior to the closing date. Operator's Report of Motor Vehicle Accident Use this form to report a motor vehicle accident. Mail or Deliver one copy to the local police department in the city or town where the crash occurred. Mail one copy to your insurance agency. Mail original to the RMV. Don't forget to keep a copy for your records.
